fix: thought format & filename safety\n\n1. Thoughts now displayed OUTSIDE the blockquote (clear separation)\n - Original text: inside > blockquote with timestamp\n - User thoughts: plain text below the quote\n\n2. Filenames: replace () [] with fullwidth （）【】\n - Markdown link syntax [title](file) breaks on filenames\n containing ( or ), causing broken links in index\n - Now all filenames use safe fullwidth variants\n\n3. Old files with English parentheses cleaned up\n - All index links verified: 103/103 working
